RT-Extension-Survey?

Mentions as one of the features listed:-

" * Satisfaction surveys"

Accept end-user feedback and store the responses right on the associated support ticket with the free RT-Extension-Survey.

But I can’t seem to find any mention of this extension in any of the CPAN or Github Repositories.

Anyone know where/how I can obtain it?
