Can't add attachments to correspondence or comment from web interface

Just upgraded from 4.0.18 to 4.2.6 - no problems reported during upgrade
or database upgrade.

When replying to or commenting on an existing ticket from the web
interface it is not possible to add an attachment;
Browse, select a file and the name shows up next to the Browse button
under the text entry window but when I click on “Update Ticket” no file
is attached
If I click on “Add More Files” rather than “Update Ticket” the text
entry box disappears momentarily and when it comes back there is no file
attachment listed and “No file selected” next to the Browse button.

Creating a new ticket from the web interface with attachments works fine

Doing a reply with attachments on my test server, which was upgraded
from a copy of the same code base and database, it attaches files just fine.

I have not been able to find what is different between the two instances
and the logs written to the RT log file are the same for both servers,
this is what I get when Browsing for a file and clicking on Add More Files:

Google has not proved to be my friend in finding a solution for this one.

I would appreciate any ideas that you have on what the problem could be
or where I should look.
